REGISTERING TO TEST

U.S. MAIL REGISTRATION

You may register by mail during the regular registration period only. Please note that registration deadlines are strictly enforced.

Your registration form and payment must be in the same envelope and must be postmarked by the regular registration deadline. If your registration bears multiple postmarks, the most recent date will govern. Registration materials postmarked after the regular registration deadline will be accepted automatically as registration for the next test date. If you are automatically registered for the next test date, you will receive a refund for any overpayment approximately six weeks after the test date.

Before you register. Before you register, please review the “Registration Checklist” to ensure that you have all of the information you need to complete the registration process. Because of laws protecting confidentiality and privacy, only you may register yourself for a test.

If you wish to request alternative testing arrangements, please submit your request and any required documentation by mail with your registration form, or fax it separately, before the regular registration deadline. See “Registering for Alternative Testing Arrangements” for information about how to complete and submit the Alternative Testing Arrangements Request Form.

How to register. To register, complete the registration form by following the instructions listed on the form. The registration form can be found in the envelope in the printed registration bulletin and can also be ordered from the THEA Web site at www.thea.nesinc.com by selecting “Print Bulletin and Forms (PDF).” Enter your name as it appears on your identification. The registration form is processed by computer; therefore, a photocopied or faxed registration form will not be accepted.

If you do not have a social security number, use the identification number assigned to you by your institution. If you have not been assigned one, leave section 5 of the registration form blank. Evaluation Systems will assign an identification number to you that will appear on your admission ticket. Each time you register, please use the identification number assigned to you by Evaluation Systems.

After you have provided and checked all the information on the registration form and then signed the form, mail it along with the correct payment in the envelope provided (see “Mailing the Registration Form” below). If your form is received without your signature, it may be returned to you unprocessed. See “Test Dates” for the registration deadlines associated with each test date.

Payment. Payment must be made at the time of registration by money order, cashier’s check, or personal check. See “Test Fees and Payment Policies” for complete information about registration fees and payment policies.

Confirmation. You will receive your admission ticket by U.S. mail after you have been assigned to a specific test center. If you have not received your admission ticket by one week before the late registration deadline, call (866) 565-4879.


Mailing the Registration Form
  • Your registration form and payment must be sent in together. Your registration form cannot be processed without payment.

  • Please check the accuracy of the information you provided on the registration form, especially your social security number. Remember that the forms are machine processed; if you fill in the ovals incorrectly on your form, that information will be recorded incorrectly on your records.

  • After you complete and check all the requested information on the registration form, be sure to sign the form, write out the identity certification state­ment, and place the form in the envelope provided.

  • Include your money order, cashier’s check, or personal check for the correct amount, payable to Evaluation Systems. Make sure that you have included the last five digits of your social security number on the check or money order.

  • If you require alternative testing arrangements or an alternative test date, enclose the appropriate documents.

  • Mail your completed registration form and payment in the envelope provided to Evaluation Systems.

Notice of Error in Registration

If you do not sign your registration form or you make an error that prevents your registration from being fully processed, your registration materials will be returned to you unprocessed with a letter describing the error and explaining how to correct it. This may delay your administration date or may cause you to incur additional registration fees.

Your corrected materials must be postmarked by the regular registration deadline in order for you to be registered for the current administration. The postmark deadline is strictly enforced. If you mail your form and payment after the regular registration deadline, you will be assigned to the next test date.

If you miss the regular registration deadline, you may choose to register on the Internet during the late or emergency registration period.

If you owe any fees from a previous administration, whether you attended the administration or not, you will not be permitted to register for or take the test until you clear your account. Any payments received for a future test date will be applied to your outstanding balance. You will be notified in writing if your registration fee and materials are applied to an outstanding balance. In order to register for the current administration, you must submit another registration form and payment to be postmarked by the regular registration deadline. If you miss the postmark deadline, you may consider using the late or emergency registration options. All late or emergency registration fees and rules will apply.
